See Below for Detailed Information Observation: Agency self-reported that a four (4) year old child had been left on the front playground alone for a period of approximately one (1) minute when the educator of the class had transitioned through the gate to go inside after outside play. 1240-04-01-.11 SUPERVISION Plan of Corrective Action: Agency will ensure that name to face roll call will be completed during transitions from inside to outside and outside to inside. Agency received training from CCR&R which gave suggestions on transitions and roll call.

1240-04-01-.12 (13) (a) (3) Smoking/vaping is not permitted within fifty (50) feet of the child care agency entrance. When LC Collier pulled into the parking lot, an educator was observed walking into the building along the sidewalk by the parking lot. This educator had a vape pen and used it approximately 3 times before walking into the building with the vape pen. This sidewalk is approximately 6 feet from the building and within 50 feet of the entrance. The educator's personal belongings were locked in a cabinet, inaccessible to children. LC Collier discussed adding a policy to prohibit tobacco/smoking products being allowed in the building. The agency has ample No Smoking/No Vaping signs posted on the building in multiple places.

Observation: Three (3) children were not signed into the program and two (2) children were not signed out of the program as required with times in/out and parent printed names and signatures. 1240-04-01-.11 SUPERVISION. Corrective action plan: Agency has posted new signs to remind families about signing in/out. All families were given a letter about proper sign in/out procedures, the importance of accurately recording attendance, and the steps that will be taken if they are not complying with procedures. All educators were retrained in procedures for ensuring that all children were signed in and out.
