Baby wipes and chemicals in the infant classroom are out of reach but are not locked up. Items labeled "keep out of reach of children" must be locked away to achieve quality points. Summary of Complaint Investigation #PCS1002213: LC Collier investigated all issues and conducted interviews with Director, Assistant Director, and Educators. A referral to CCR&R will be made for Inclusion Services. Rule Sections Monitored: Health and Safety Critical Items Other Monitoring/Observations: 1. Age Groupings: LC Collier checked the ages of the children in each classroom. The preschool classrooms were all grouped appropriately. LC Collier discussed with the director the ages of preschool classrooms. LC Collier discussed using a developmental checklist and sharing it with the families when determining if children should promote classrooms. A transition plan is required and open communication with the family is imperative to make the best decision for a child. 2. Meal Service: LC discussed the agency's meal service and asked what the center would do if a family requested special accommodations for meals. The director will request a doctor's note for special needs for children that may need special food. The center had ample documentation of doctor's notes requesting special food accommodations. As children age, LC suggests updating their developmental history yearly in order to have the most accurate and up-to-date information for children's meal preferences and capabilities. (example: Likes, dislikes, stomach issues, sleeping patterns, etc.) 3. Discipline: Upon an interview with the director and educators, redirection and time-out in a cozy center are most commonly used. LC discussed using time-out and ensured the educator is aware of the rules regarding time-out. *Rule 1240-04-01-.15 (5) Time-out may be used to intervene with a child whose behavior is disruptive to the group or hurtful to other children and who does not respond to educator redirection or guidance. 1. Time-out shall be reasonable and developmentally appropriate and shall not include restraint or seclusion. 2. The length of each time-out session shall be based on the age of the child and shall not exceed one (1) minute per each year of age of the child; provided, however, that no child under thirty-six (36) months shall be placed in time-out. 4. Lack of Cooperation with therapists: The center has asked one group not to return to the center due to the therapist not complying with agency policies. The center does allow therapists from other agencies and now has stipulations in place to ensure therapists follow their policies. When therapists are working with children in the center, LC Collier recommends requesting documentation of what and why the child is receiving services for. LC also suggest ensuring a staff member is with a child if a therapist takes the child into a separate area for services or having the parent add the therapists to their authorized pick-up list. 5. Staff personal belongings should be inaccessible to children. One educator's purse was accessible and this was corrected while on site. No Violations
