OBSERVATION: LC noted on the day of the re-evaluation that five children's records did not have a physical on file. 1240-04-01-.08 RECORD KEEPING. Plan of Corrective Action: Agency will be getting a copy of the child's physical from the parent and attach it to the child's immunization record.

OBSERVATION: ON 2/17/2022 it was noted that there were eleven children present at the agency. Seven children were signed in by their parent/guardian. Four children were not signed into the agency. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: The agency will be reminding parents/guardians before leaving the facility to sign their child out of the facility. The next step will be that they will be calling the parents/guardian to come back to the agency to sign their child in if agency notices it after the parent/guardian has left the premises.

Observation: During the assessment, an infant was not checked by physical touch every 15 minutes as required. 1240-04-01-.11 SUPERVISION (Safe Sleep) Plan of Corrective Action: Technical Assistance provided by Program Evaluator and retraining for educators for SIDS. Corrective Action: The assessor provided technical assistance during the assessment and the Program Evaluator made a referral to Child Care Resource and Referral.
