Observation: Agency enrolled a child under the age of 36 months (three years old), when their agency is licensed for children 3 years to 12 years. 1240-04-01-.11 SUPERVISION Corrective Action: Agency immediately placed him in the agency which is licensed for 6 weeks to 3 years, owned by the same person. Owner will make a list of the children enrolled, oldest to youngest, to monitor that the youngest child is 3 years old.
