The staff was asked to place the child on his back and she did immediately. Violation: 1240-04-01-.11 6)a 3 Supervision (safe sleep) Plan of Corrective Action: Director will review safe sleep with all staff, and to insure that staff remember will place safe sleep posters in the area that the children sleep.
