Observation of violation: After speaking with the agency owner, agency director and an educator for the two-year-old classroom, it was determined that a child had exited the agency unattended. Violation: 1240-04-01-.11 – Supervision Plan of Corrective Action: The agency owner will put “alarm” type instruments on each of the doors of the classrooms leading outside of the agency. The owner will also work with CCR&R to receive training on “runners” and other types of supervision.

Three incidents of biting involving the same two children have occurred in this area in a short amount of time showing inadequate supervision being provided.
