The supervision lapse lasted 16 minutes and when the teachers noticed the child was not with the class on the playground, one of the teachers went back to the classroom to find him. 1240-04-01-.11 (1) (a) Supervision Procedures Corrective Action: The teachers have been re-trained on roll call procedures to prevent further supervision lapses.
