See Below for Detailed Information Observation: (1) While viewing the video footage of the incident on 4/22/22, the educator was focused on other tasks and not supervising the child who climbed onto the table and fell. The educator was observed walking past the child as he attempted to climb on the chair without redirecting. (2) While viewing the video footage of the incident on 6/6/22, the educator was observed cleaning up the classroom and not actively supervising the children. Rule: 1240-04-01-.11 (1) (a) Corrective Action: The teacher will be required to undergo more supervision trainings in order to return. The door is to remain closed to ensure better visualization of the children during diaper changes. Also, a floater will be made available to assist during each diaper change. Observation: On the video footage, the educator was observed using the emergency evacuation crib as a restrictive device for the children. Three children were in the crib while the educator cleaned up the room. This was not being used for the intended use by the manufacturer. Rule: 1240-04-01-.14 (1) (c) Corrective Action: Our center will no longer use the Emergency Crib for transporting children. Any teacher needing assistance transporting children will need to call for a floater to assist them with the transition. We will also have walking ropes available for the appropriate classrooms. I will be assessing each class to assure we are using the best and safest option for transitions. Observation: On the video footage, the three and four children were observed in one crib which was behind the open door. The children were left unattended in a restrictive devices. Rule: 1240-04-01-.15 (i) Corrective Action: I will be refreshing all staff of the importance of comforting children when they are upset, no matter how small; the children's feelings should always be recognized. I will also be in contact with CCR&R for possible trainings that may be offered to each staff member.
