Observation: Camera footage showed that a three year old was left on the playground without the knowledge of the staff. 1240-04-01-.11 (1) (c) 3. Supervision Corrective Action: All staff will receive training through CCR&R on supervision (both indoors and outdoors). The gate/fence will be made more secure so that children are not able to open the door by themselves.

The camera footage showed that the child was left on the playground. After being left on the playground he opened the gate to the playground and walked to the building door. The teacher did not notice the child was left on the playground which caused the supervision lapse. The teacher was also not actively watching children. When reviewing new staff files one staff was missing the shaken baby training. 1240-04-01-.06 - Staff Plan of Corrective Action: To make sure that the right training is done. This staff did the wrong training, she did SID and not Shaken Baby. In the future the director will make sure all staff have the right training before working with the children.

Observation: During the interview of staff, it was noted that some staff focus on unacceptable behavior with some of the children. Corrective Action: Agency states they plan to address this with staff members about when children are having issues to not solely to focus on the unacceptable behavior. Director will get work on getting some tools for the staff to have when working with these issues. CCR&R is also doing training with all staff on these matters.
