On 2/6/24, for the purpose of a Complaint Investigatatoin, LC Collier conducted an unannounced visit. During the visit, the agency did not provided adequate documentation that the disenrollment policy was followed.

A safety plan is in place at the agency. During the visit, the individual on the safety plan was properly supervised and supervision logs were maintained. LC Collier observed DCS Gail Bowers as interviews and investigations were conducted. Camera footage was reviewed and will be emailed to DCS Bowers by Director Lawson. Check Sheet reviewed with Director. Technical Assistance: LC Collier will be available to the agency as needed or requested. LC Collier reviewed the center's Personal Safety notification procedures and advised to ensure all parents are notified and that documentation is kept on-site. LC Collier reminded the agency to follow their disenrollment policies and licensing rules during the complaint investigation. No violations noted.
