See Below for Detailed Information Observation: An educator was observed using inappropriate discipline on a 3 yr. old child during naptime. 1240-04-01-15 PROGRAM, LANGUAGE AND LITERACY DEVELOPMENT Corrective Action: The agency provided documentation of the details of the incident that occurred on 11/19/18 when an educator used inappropriate discipline. The decision was made to terminate the employee on the same day. PE was made aware of the incident on 11/14/18 and made an unannounced visit to investigate and returned on 11/16/18 for a follow up and to amend the check sheet on 11/14/18 to add the violation.

Observation: Agency had three (3) out of seventy-nine (79) children in attendance today that were not signed in and/or out by the Parent/Guardian or Authorized Representative. 1240-04-01-.11 SUPERVISION. Corrective Action: Parents or other persons dropping off and picking up children are required to note time, print and sign name on the pre-printed sign in/out sheet for their child located in the foyer. Parents will be called to return to the center if they fail to sign in/out at the time of drop off/pick up. A member of management will check the sign in/out book throughout the day to assure that all children are properly signed in/out. A member of management will explain the sign in/out procedures at the time of enrollment. We have put reminders in our parent news letter and have posted reminders on the front door also by the sign in/out book to help parents remember to follow procedures. Parents have been made aware that failure to sign in and out each day of attendance will result in a $25.00 fine for each occurrence and the possibility of disenrollment from the program.

Corrective Action: Staff will sign all children into the classroom as they arrive each day and out as they depart for the day. The KinderCare CSR (Child Supervision Record) will be used for this documentation along with the KinderCare App, Roll Call Sheets and Meal Count and Attendance Sheets. A member of management will check documentation throughout the day to assure that documentation is up-to-date and correct. Staff were retrained on supervision and what the expectations for supervision are moving forward. Staff reminders will be noted monthly in the staff memo. Staff not following proper supervision guidelines will be reprimanded, which may include termination. New hires will be trained on supervision during their new hire orientation and made aware of the importance of proper supervision. Management will be responsible for the training and the implementation.
