Corrective Action: The agency should have made sure that the cook complete the required disclosure form before proceeding in her hiring process. The agency should have made sure that they had a Clearance letter for her to work around children before allowing her to begin cooking and being in the classroom.

Observation: Management hired a cook that did not fill out her disclosure form or sign it and then hired her to begin work in the kitchen before a disclosure letter came back. She has been cooking and bringing the food into each classroom around the children. 1240-04-01-06 Staff Corrective Action: The disclosure letter should have been filled out and signed by the potential hire and a clearance letter should have been in the agency's possession before they let her work in the presence of children.
