The agency failed to report the concerns to the Department of Children's Services immediately as required by Tennessee Law and Licensing Rules & Regulations. On November 12, 2024 concerns of improper discipline techniques was reported at this agency with possible incidents occuring Nov. 8, 2024 The agency failed to report the incident(s) to the child's parent/guardian or authorized representative or to the appropriate regulatory agency immediately on the date of occurance.

Observation: During the Assessment conducted on 10/23/18, two supervision lapses were observed in the 3 year old classroom by the Assessor. 1240-04-01-.11 SUPERVISION Plan of Correction: The agency implemented a plan of action for the supervision lapses that occurred 10/23/18 the same day. Transition procedures were discussed with the caregivers to address the issues of students lagging behind or running ahead of the teacher. Head counts and roll calls will be conducted before they leave an area and move to another area. The caregivers will read and sign the new procedures. The director will be observing and monitoring this classroom to make sure these students are being properly supervised.
