Summary of Complaint Investagion and Safety plan Served Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint and serving safety plan. During an unannounced visit on March 14, 2024, for the purpose of a complaint investigation, Child Care Licensing Consultant (CCLC Carmon) validated an agency self-reported incident by reviewing video footage from March 13, 2024, around 12:00PM. CCLC Carmon observed on the video footage that the educator did roll call outside the classroom after lunchtime. The educator opened the door and the children went inside the classroom. See Below for Detailed Information Observation: Child, aged four (4) years, left unsupervised in the Dobbins E classroom for four (4) minutes and sixteen (16) seconds on January 26, 2022. 1240-04-01-.11 SUPERVISION. Corrective Action: Educators will complete one on one training with CCR&R on February 11, 2022 and will agree to use the techniques provided by CCR&R. Training Team Leader will review and retrain Educators on Safety and Supervision – Active Supervision Procedures (In-Service, February 28, 2022), UETHDA’s Standards of Conduct, Head Start Performance Standards (HSPS) 1302.90(c)(1)(v), and TDHS Licensing Regulations. Educators will agree to follow all Active Supervision Policies and Procedures. Education Manager had a conversation with Educator involved about developing additional measures to ensure compliance, which include (1) dots in hallways for children to line up for roll call and counting (2) checklist of children present for calling roll in addition to sign in/out log and (3) roll call will be conducted when transitioning out of the classroom. Each year UETHDA’s Standards of Conduct is reviewed with all Head Start staff and they sign that “I have read and understand the contents in the Standard of Conduct Policy of UETHDA. I agree to comply with all provisions of the Standard of Conduct during my Contract with UETHDA.” The Standard of Conduct Policy states that “Leaving a child alone or unsupervised while under the program’s care,” is considered an unacceptable activity.
