After conducting an unannounced visit on 04/22/26, LC Walker and Sanders determined on 4/20/2026 there was one educator to 11 children in the 2-3 year old room with 8 children under 30 months.

Summary of QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, scoring of the QRIS and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. N/A Staff did not finish the required trainings by the end of the QRIS year. There are several staff that does not have documentation that staff has been screened or tested for communicable diseases. In the infant classroom there were no materials accessible such as books, fine motor, blocks, dramatic play, etc. to the children that were nonmobile. Educator stated they were learning about farm anaimals and there were no materials relevant in the class that farm animals were being taught. Staff did not complete required training for the previous 2024-2025 QRIS year. Due to Management responsibility, the Director did not make sure that the staff completed the required training for the 2024-2025 QRIS year Director falsified training documents for twelve (12) staff for the previous 2024-2025 QRIS year.
