Viewed video from 1/24/24 Educator is witnessed taking a child by the upper arm and pulling her into a different spot on the floor for circle time. This child is handled more harshly than is appropriate. This child does get mad/upset after being moved. Educator is witnessed using her foot to move the foot of another child. Educator is also witnessed using her index finger to tap a child in the mouth. Educator was also witnessed picking up a child (this is the autistic child in the classroom) and putting him in the playpen (it has been approved for this child to be placed in a playpen by the parent and DHS). This child is periodically placed in a playpen due to him getting stimulus overload and needing a break from other children. Age-appropriate toys that are developmentally appropriate for this child are also placed in the playpen. When the child was placed in the playpen he could have been picked up more gently and placed more gently into the playpen.

Observation: Program Evaluator observed an educator in the Toddler classroom without a clearance letter on file. This educator had been in the classroom since the morning of 10/9/2020 and the visit was at 2:13 pm the same day. 1240-04-01-.07 Criminal Background Check and State Registry/Records Review Procedures. Corrective Action: The agency will ensure that all staff have official OK to work letter on file before allowing them in the classroom in the future.

Observation: During an unannounced visit on 7/11/19, Child Care Program Evaluator (CCPE) Jones observed that the educator in the 12-30 month classroom was at a 1:10 ratio at nap. The ratio for this age group is not relaxed for nap time so the ratio should have been 1:6 1240-04-01-.22 Specific Requirements for Child Care Centers. Corrective Action: The assistant director called an educator back from lunch and will stagger lunches in the future.
