See Below for Detailed Information Observation: An educator left children in the infant classroom to come to the door to let an individual in, and an educator had also left one of the rooms to start lunch. 1240-04-01-.11 Supervision Corrective Action: The violation was corrected on-site when another educator returned back to the agency. Observation: An educator left children in the infant classroom to come to the door to let an individual into the agency. An educator left a classroom to start lunch. With both incidents, classrooms were left out of ratio. An educator also had children as young as one year old grouped with children as old as four years old in the classroom. Another educator had a one year old in the 3 to 5-year-old classroom, and another educator had children as young as infant grouped with children as old as three years old. The children appeared to be able to go into any classroom. 1240-04-01-.22 Specific Requirements for Child Care Centers Corrective Action: Violation was corrected on-site when another educator arrived back to the agency and children were moved back to their original classrooms.

See Below for Detailed Information OBSERVATION: On 3-26-18 it was observed that an infant was sleeping in a soft bouncy seat. This violates the safe sleep/soft bedding rule. 1240-04-03-.06 SUPERVISION/Safe Sleep, SIDS The issue was immediately corrected by the caregiver moving the baby to a crib. The Owner also completed a Plan of Corrective Action in which she stated she was going to retrain all of her staff on safe sleep procedures. OBSERVATION: On 3-26-18 it was observed that the infant room at Little Treasures was out of ratio and needed an additional caregiver. The ratio was 1:7. 1240-04-03-.06 SUPREVISION/Adult:Child Ratios CORRECTIVE ACTION: The violation was corrected on site with an additional caregiver being called in to work. The Owner/Director of the agency was also required to complete a Plan Of Corrective Action, which she did. A temp agency will be utilized to ensure a greater level of dependability.

Plan of Corrective Action: Notification of all accidents, illnesses, and injuries will be reported to parents effective immediately. Seven preschool children were left without supervision while napping when the caregiver left for lunch prior to another caregiver taking her place. 1240-04-03-.06 SUPERVISION. Plan of Corrective Action: Children will be supervised at all times, including naptime, effective immediately.
