Summary of Complaint and Serious Incident Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and the investigation of a complaint and self-reported serious incident. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The agency self-reported that on 5/10/2024 a 2-year-old child had unlocked a child-proof lock on the door of his classroom and left the building. Upon reviewing the agency's videos and interviewing the director, two educators, the parent of the child, and the parent who found the child outside, it was determined that the child escaped the agency and was unsupervised for 2.5 to 3 minutes. The child traveled from the front of the building to the back covering approximately 35-40 yards. Upon reviewing the agency's videos and interviewing the director, 2 educators, the parent of the child, and the parent who found the child outside, the Licensing Consultant determined that on 5/10/2024 at approximately 4:45 pm, a child escaped from a preschool classroom and was unsupervised for 2.5 to 3 minutes. The educators were unaware that the child was not in the classroom until informed by the Director.
