(h) Following the completion of pre-service transportation training, all persons responsible at any time for the transportation of children (including drivers and monitors), shall complete Department-recognized transportation training on transportation rules every six (6) months. (i) Emergency Aid Training. All persons responsible (including all drivers and monitors), or who in the course of their duties may become responsible at any time, for the transportation of children shall hold current certification in: 1. Infant/Pediatric Cardiopulmonary Resuscitation (CPR) from the American Red Cross, the American Heart Association, or other certifying organization as recognized by the Department; and 2. A first aid course sponsored or approved by the American Red Cross, or other first aid course, as recognized by the Department. During a review of the agency's driving staff files, there were several drivers who needed a copy of an updated license on file, documentation of updated CPR & First Aid certification, drivers who needed documentation of updated physical exams on file and updated DHS Transportation training was needed for driving staff. *(one driver had no documentation of having received DHS Transportation training on file). One driver was driving the agency vehicle although she had a DHS Investigations letter dated 10/2012 that she was "Not OK to drive".

Corrective Action: Management will schedule and prepare for additional staff to be onsite to provide coverage in case of an emergency or unexpected situation in order to ensure adult:ratios are compliance at all times. Observation: Children were transported on a bus with expired annual DOS inspection effective 11-16-18. 1240-04-01-.17 TRANSPORTATION. Corrective Action: The agency will have the vehicles inspected prior to transporting children. The agency will put a system in place to ensure all required maintenance, tag registration and inspection is renewed prior to the expiration annually.
