  During an unannounced visit to the agency to investigate an incident/injury and complete QRIS monitoring, LC reviewed the incident documentation and determined that the injury occurred on Tuesday, 05/26/2026, at approximately 4:59 p.m. The incident was reported to DHS on 05/27/2026. Therefore, the incident was not reported to DHS on the date it occurred.

An educator was witnessed by both staff and parents using inappropriate language in the presence of children.
