The building was not safe and in good repair. Adequate lighting was not provided in the boys nor the girls bathroom. There was a hole in the baseboard behind the toilets in the boys bathroom. The ceiling light covering tile was hanging from the ceiling in the boy's bathroom. The girl's bathroom was dark and the children's faces could not be clearly seen. A ladder was leaning against the wall in the girl's bathroom. On the playground, there was a little tyke vehicle that was broken and busted with the steering wheel broken. The climber/slide did not have sufficient surfacing. You could see the grass coming through the pea gravel and there were splintered wood that should be replaced on the hummer vehicle and the standing base of the slide. There were several lights that were not working throughout the building which cause the light in the classroom areas to be dim.
