Observation: During a virtual visit, it was discovered that a staff member started working in the facility prior to receiving clearance from fingerprint and abuse registry background checks. 1240-04-01-.7 Background Check Plan of Corrective Action: Make sure Director/Owner will assure all staff complete criminal background process and be cleared to work before assuming duties. Plan Of Corrective Action Statement by Director: Background checks will always and have been cleared before hire date. I understand now that all out of state employees have a two part clearing process. This method of clearing will be recognized and made more aware of. Record keeping will always maintain an up to date status.

The agency had received a complaint regarding an 8 month-old infant falling out of a bumbo seat during play time. When I made an unannounced visit to the center, I observed the bumbo seat and the seat did not have straps on it to properly secure the child during play time. Therefore, the equipment manufacturer's safety instructions were followed; neither was it safe for the infant. [Equipment-1240-40-3.08]
