See Below for Detailed Information Observation: The daily sign-in and sign-out sheet does not include the full printed name of each child nor a space for the print name and signature of parent/guardian or authorized representative. SUPERVISION. 1240-04-01-.11 Corrective Action: Owner will create a form that includes a space for the printed name and signature of the parent/authorized individual and begin using it on 04-23-2019. OWNER will spend time reading the rules and will conduct a training with staff to review new rule requirements. Observation: New educator file is incomplete with no documentation of completing Shaken Baby Syndrome/Abusive Head Trauma and Before You Begin pre-service training prior to having contact with children starting 01-22-2019. 1240-04-01-.06 STAFF. Corrective Action: Educator stopped providing child care duties and began registering online to complete training in Before You Begin and Shaken Baby Syndrome. Owner will review staff files once every 3 months and will not allow new hired staff to have contact with children prior to submitting and completing all employment requirements. PE shared the licensing rule. Observation: Sign in and out sheets for April 12, 17, and 18, 2019 have four (4) children that are not signed out of the agency. 1240-04-01-.11 SUPERVISION. Corrective Action: All errors on logs were noted and discussed with Owner. OWNER report she will review the logs each day to monitor the logs to ensure that each parent sign in and out of the agency with the time, printed name and signature at arrival and departure. PE recommended logs be reviewed more frequently to catch and correct any errors.
