Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint. Technical Assistance Provided: 1. LC discussed the rules section of all violations.LC made a TAMS referral for training on supervision and health & Safety for all staff. Safety plans will be put in place for the agency and all staff that were on the playground. All staff confirmed they understand the rules and are receptive of the TA. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During the unannounced visit, LC observed two children in the bathroom unattended. One of the children was requesting assistance and no one was there to assist. During interviews LC discovered that all staff with the exception of the staff that was inside preparing lunch and the staff that was engaging the assigned children to him, were under the covered patio outside feet away from the gate that the child exited. The dtaff were not monitoring all of the children as the child was able to get out of the gate in thier presence. LC observed the gate that the child opened to exit the playground. LC observed that the latch on the gate could be easily lifted to open the gate and exit the playground. During the unannounced visit, several educators reported that everyone was on the playground during outdoor play. The educators reported that all of the educators with the exception of one other that was on the playground and one in the agency, were sitting under the covered area while the children were playing. Educators reported that none of them were aware that the child had exited the gate. During the unannounced visit, LC viewed the childs file and there was no incident report of the incident that occurred.

CI 1.11: Background Checks
There are three employees that are working that are not cleared for this location. The three employees are cleared for the Family Day Home but not the Center. LC contacted Criminal Background Inqquiry and was informed that the althought the initial fingerprint was done at the other location, the transfer form would have to be completed for the 2nd location. The provider stated previously that the form was completed to show that the employees were to be active for both locations. Criminal Background Inquiry could not verify forms being submitted and stated that only the inital fingerprint was done but at a different location.
