See Below for Detailed Information Observation: An assigned educator hit a two year child on the hand to correct the child behavior. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The educator resigned before being terminated on 12-02-19. Another educator is now assigned to the class replacing the resigned educator. PE requested training from CCRR regarding discipline. On 12-11-191; CCRR representative conducted Appropriate & Inappropriate Discipline training with staff. Documentation was emailed to PE. Observation: There is no incident report documentation, of the agency, reporting the incident to the parent of the child that was hit, by the assigned educator. Rule Cited: 1240-04-01-.09 INCIDENT REPORTING. Corrective Action: The educator resigned before being terminated on 12-02-19. Another educator is now assigned to the class. The parent of the child hit was notified. PE requested Director document and provide a copy of PE as verification of corrective action taken. Documentation of Director meeting with staff to review the Behavior Management Policy and Licensing requirement regarding hitting and spanking children.

See Below for Detailed Information Observation: During the annual assessment, on October 24, 2019, infant educators, failed to touch a napping infant every 15 minutes. 1240-04-01-.11 SUPERVISION. Corrective action: CCRR was called onsite and completed training with staff. Director reviewed Supervision regulation training with staff. PE provided technical assistance by sharing FSI idea to touch the child and document safe sleep touches prior to turning the alarm/timer off.

The sign in and out sheets for April 08, 2019 have one child not signed out. The sign in and out sheets for May 14, 2019 have one child not signed in but showing signed out. The sign in and out sheets for May 16, 2019 have two children not signed out. 1240-04-01-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: The sign in and out sheet was noted for correcting upon return of the parent or guardian. Violation: Individual class list are not being maintained in each class. 1240-04-01-.08 RECORD KEEPING. Corrective Action: A blank piece of paper was given to each educator to date and create a list of all children present in each class. Observation: Information on feedings, spitting up, diapering, sleep patterns and developmental progress is not being documented for all infants and non-verbal children. 1240-04-01-.08 RECORD KEEPING. Corrective Action: A copy of Infant Daily Report forms and Daily Child Care Health Check sheets were provided to each educator for each child present. Educators were advised to begin documenting the information daily and immediately.
