During an unannounced complaint investigation visit made on 1-23-25, the Licensing Consultant observed the adult:child ratio to be 1:8 in the 12 - 30 months class with the one educator. There was no other educator in the class with her during my observation.

During the re-evaluation visit, upon review of staff's records it was discovered that an employee was working in the infant classroom without an Okay to work letter. Observation: PE observed A lock was placed on the doorknob in the 2-year-old class and gate was still up in the Toddler classroom; after PE asked the agency to take it down because its blocking the exit. All these locks on the door and the gate is a fire hazard. 1240-04-01-.16 Physical Facilities Corrective Action: Provider will ensure that no gates are placed in front of exit doors and no locks are placed on the door knobs.
