Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. On November 11, 2025, Licensing Consultant (LC) viewed video footage of a reported incident that happened at the center on October 24, 2025, that involved a child causing harm to another child while the educator was in the classroom with her back turned to the incident. The educator failed to turn around for a significant amount of time, even while an attempt to notify her was made by one of the students. Licensing Consultant (LC) conducted an unannounced visit on November 3, 2025 at 10:30 am. During the visit, LC viewed files of the children named in the complaint. While viewing the files. LC noticed that an incident report was missing for one of the mentioned incidents, in one of the children's file. The incident report was only in one of the children's file.
