License Consultant observed an infant holding and drinking a bottle while educator pushed the stroller. A mealtime supervision plan was not observed

Swing and slide set was not securely anchored. License Consultant (LC)observed shaking and rocking of swing set while children played on the swing and slide. LC observed loose screws on swing and slide set. The floor board on top of slide was loose and the boarder around the swing and slide set was not secured. The would chips and mulch measure between 3-4 inches around climber slide. LC observed a staff purse located in a child's cubbie and hand sanitizer attached on end of purse.

During the announced visit on 11/18/2024, the Licensing Consultant (LC) observed the swing and slide set was not securely anchored while the children were playing. LC also observed loose screws and bolts on the swing and slide set. The wooden floor board at the top of the slide had loose wooden planks. The wooden border that surrounded the swing and slide set was not secured. LC observed a child walking on the wooden boarder and it moved allowing the child to almost slip and fall. The wood chips and rubber mulch measured between 3-4 inches around the slide and climber wall.
