Summary of Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. It also included a review of the QRIS. Technical Assistance Provided: 1. Making sure incident/accident reports are explained and signed by parents the same day as incident 2. Make sure expulsion policy is read and sign by parents No Violations were received OR Violations were received and include: 1. Incident/Accident 2. Expulsion Policy Correct Action Plan (CAP) completed. Violation details can be observed below. On March 8, 2023, a complaint investigation was conducted about a provider contacting a parent over the weekend to terminate their child enrollment for the following week. According to the Provider she did not get the parent to read over and sign an expulsion policy upon enrollment. The Provider stated that she verbally told the parent that she would give her a one week trail due to the child circumstances and low staffing. DHS Licensing Consultant Grady requested to see the policy. The policy did not state anything about a one week trail. Compliance requires that the child care agency have a written expulsion policy and the policy shall be clearly articulated to the staff and parents. The policy shall be developmentally appropriate and consistent. The policy shall also be non-discriminatory in practice and impact. Other options shall be considered prior to expulsion, such as but not limited to reducing the number of days or amount of time the child may attend, or if applicable, referrals to a behavioral health program or other appropriate supporting program. Procedures shall be developed to allow for a planned transition of a child to another program if expulsion must occur. Aggregate data that includes reasons for expulsions shall be maintained and reported to the Department on a Department-approved form when such expulsions take place and as requested. 1240-04-01-.05:Ownership, Organization and Administration On March 8, 2023, during a compliant investigation of a child having a cut under his eye and his eye was swollen.
