LC conducted a unannounced visit on 05/22/2025 to investigate an incident/injury report. During the visit, LC viewed the file of the child mentioned in the incident. While viewing the file, there was no incident report in the file. LC asked the assistant director for the incident report and it was immediately provided to the LC. The incident report was complete, signed and dated by the parent with the date of the incident. However, the report was not in the file.

Licensing Consultant (LC) conducted an unannounced visit on May 22, 2025, to investigate an incident/injury report. During the investigation, the LC interviewed the educator, Assistant Director, and staff knowledgeable of the incident.During the interview, the educator reported that she lined the children to go outside and conducted the face to name. She reported that she conducted another face to name once the children were in the hall and counted 12 children. She stated that the child (in the incident), was 3rd in line. The educator stated that once they got to the stop sign in the hall at the door, she conducted another face to name count and counted 12 children. The educator reported that once she got on the playground, she conducted another face to name count and counted 11 children. She reported that she turned around and before entering the building she counted 11 children again. She stated that once she entered the building, she noticed the child being held by another teacher and crying. She stated that she immediately consoled him and had a conversation with him. She stated that she asked the child when he got out of line, and she stated that she children were saying he stopped and was tying his shoe. LC asked the educator is she did an incident report and notified the parents, and she stated that the parents were notified. However, she stated that she did not do an incident report the assistant director completed the incident report

The Licensing Consultant (LC) conducted an unannounced visit on February 4, 2025 to investigate an incident/injury report. During the investigation, LC interviewed the educator in the Toddlers (1-year olds) classroom who was indicated in the incident report. During the interview, the educator reported that she accidentally left a 1-year old child unattended in the classroom when she took her class outside for outdoor play. The educator reported that she called the roll and did a face to name check. However, while she was leading the children outside, the child "stopped in his tracks" while they continued outside. The educator reported that another educator noticed the child in the classroom and brought the child to her. The educator stated that the child was in the classroom about 2 minutes before being brought to her.
