Summary of Incident/Injury Complaint investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. License Consultant (LC) Tatoyra Grady was informed during a compliant investigation, that on 12/06/2024 during naptime, the toddlers in the doodlebug classroom was left unattended. The educator left the classroom to take a toddler that was awake to play in the gym. The educator placed the child in the gym with three (3) educators and six (6) toddlers. The three (3) educators was not aware that the child was placed in the gym with them. According to the interview the educators was assisting with a diaper change in the gym. They heard a child crying and thats when they saw a toddler from the doodlebug classroom standing by the door and the tip of his finger was stuck in between the door. When the educator rushed over to get him she discovered the tip of his finger was trapped between the door.
