The License Consultant reviewed the child's folder, that was involved on the complaint. The License consultant also reviewed the sign In/Out Sheets for the month that was in question (August) and the current month. The child was born June 30, 2021. After reviewing the Sign In/Out sheets, it indicated that the child had been present at the center before the child was 6 weeks. The Sign In/Out sheets showed that the child first day was on August 4, 2021, which the child would have been four weeks and six day old. Enrollment of children under six weeks of age is prohibited.

RECORD KEEPING Corrective Action: Management contacted the child mother and advised her to pick the child up due to a lack of proper immunization. Management informed mother the child cannot continue to receive child care without a certificate of immunization. The child was picked up and signed out of the agency by an authorized representative.

To ensure the violation does not occur again, the Primary Caregiver will check all children file to ensure all incidents, accidents and injuries are noted in the child file immediately.

Observation: The child care agency did not have documentation of the emergency plan monthly reviews. PE left a checksheet with the violation and Plan of Corrective actions. 71-3-517 Child Care Facility Emergency Preparedness Law Corrective Action: A plan was put in place to ensure the ER plan is reviewed monthly immediately before or after each monthly fire drill. Documentation of this review is maintained on file with documentation of the monthly fire drills.
