Child was immediately returned to the Directors office. 1240-04-01-.11 Supervision Corrective Action: Agency will be training educators again on supervision procedures. Director is meeting with all staff on a plan where educators must stand to ensure that all areas where doors are located are monitored more closely. Agency has purchased alarms for doors.

Observation: In February 2018 a teacher demonstrated inappropriate discipline of a child by popping a 22 month old child on the mouth for being disruptive. 1240-04-01-.05 PROGRAM. Corrective Action: A plan of corrective action was completed on site. Technical assistance was given to the teacher and director. by PE Garner. Additionally, PE Garner will provide rules training with the teacher. CCRR will provide behavior management training with the teacher.

Observation: PE observed open S-hooks on both playgrounds. Open s-hooks were on swings and rope swing aparatus. The rope of the hanging rope swing apparatus was frayed. Additionally a wooden fence was observed with split decayed wood. 1240-4-3-.08 EQUIPMENT FOR CHILDREN. Corrective Action: A plan of corrective action as left with the director to complete and return. The director noted she would have maintenance wrap the open S-hooks with duct tape for a temporary fix. She noted she would replace all S-hooks on both playgrounds. The director noted the frayed rope swing would be replaced. The director noted the wood would be repaired.
