Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation and serving of safety plans for three staff members. This is still an on going investigation and additional violations could still be cited. Technical Assistance Provided: 1. Guided staff through safety plan 2. The educator aggressively grabbed a two (2) year old child pulling them by the arm. Another child was pushed by both shoulders back into line. She also knelt nose to nose with another child yelling at the child using inappropriate language of “why can’t you freaken’ listen”. 1240-04-03-.09 PROGRAM. Plan of Correction: A plan of corrective action was completed on site. The educator was placed on a safety plan. The agency is receiving training from CCR&R.

Additionally the child did not have an immunization record on file as required by licensing rules. 1240-4-3-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: A plan of corrective action was completed on site. The owner/director noted she had the forms in her paperwork, but could not locate them. She noted she would locate them or have the parents bring her duplicates. The child was not present at the agency. The owner director noted the documentation would be on file prior to the child returning to care.

Observation: On March 16, 2018 PE Garner confirmed by reviewing time sheets and time cards for the past two weeks that on March 8, 2018, a staff member was alone in the three (3) to five (5) year old classroom with a ratio of 1:14 from 8:10 am until 8:27 am. The ratio in this classroom should have been 1:13. 1240-04-01-.06 SUPERVISION Corrective Action: A plan of corrective action was left with the owner/director. It was discussed with the director that she will ensure that ratios are met at all times.
