RULES: 1240-04-01-.11 SUPERVISION- To ensure the health and safety of all children enrolled, the management of the child care agency shall maintain a system that enables all children to receive a level of supervision that is appropriate to their age and their developmental, physical and mental status. The child care agency staff shall know where children in their care are at all times, be aware of their activities, and be able to intervene appropriately. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS-Required Adult:Child Ratios. Observation: Staff/Child ratio was 3:66 during snack. Ratio for this age group is 1:20. When brought to the Directors attention that they were over by 6, she informed LC Kristen Malone that a staff member called out last minute, and another sub could not get there in time. Children leave quickly, so by the time a sub came, they would be in compliance.

Observation: While investigating a serious injury complaint, the PE noted on video footage that the educator was not properly monitoring children's play activities. It was noted that a ten year old child was swinging two six old children by her arms around several times and releasing them causing them to slide them across the floor several times while the educator sat on the bleachers and watched. The child care agency staff shall be aware of children's activities and able to intervene appropriately to ensure the health and safety of children at all times. 1240-04-01-.11 Supervision. Corrective Action: Educators completed mandatory training on supervision and best practices.

Observation: PE Garner confirmed on 6/29/18 that there was a change the Scope of Operation when on 6/4/18, Kid's Place Seymour Intermediate (a full year program which transports) moved to Kids Play Seymour Primary (a nine month program with no transportation). The agency moved the enrollment of the program with out notifying licensing of the change of scope of the operation. 1240-04-03-.03 BASIS FOR ISSUANCE OF A LICENSE.
