Site Assistant-Daniel was observed by assessor letting a six year go to bathroom alone out of site of caregiver and thus without supervision on 5/18/09. Daniel was also observed on 5/19/09, letting children out of his sight while transitioning children from site room to snack room. A child was physically removed to another room and Daniel never observed that a child was missing as he was unable to see child in hallway when she was taken to another classroom. Amanda was observed by assessor on 5/18/09, letting children leave from inside bathroom and return to playground which involved going down hallway and through gym to outside and crossing driveway without supervision. 1240-4-3-.11 FOOD (1) Meal Service (g) Adults shall sit with children at mealtime. (o) Children shall never be left without adult supervision while eating. Site Assistant-Daniel did not sit with children and was observed retrieving paper towels and general clean-up instead of keeping his focus on children while they were eating.
