Menu was not changed to reflect what was served. 1240-04-01-.13 Food and Food Service To correct: Provider will plan out snack served for the week and make changes to snack menu prior to children arriving.

Observation: During snack, Program Evaluator Rucker observed the Site Assistant Director walk away from the tables where the children were eating. The Site Assistant Director walked behind a set of cabinets to get extra snacks. She could no longer see the children seated at the tables eating. 1240-04-01-.11 Supervision Corrective Action: Provider will position staff where they are facing the children at all times.

Observation: During the visit on January 24, 2019, Program Evaluator Rucker observed 78 children present with 3 staff. As required by licensing, the ratio for ages 5-12 years old is 1:20 and the classroom was over capacity. 1240-04-01-.22 Specific Requirements for Child Care Centers Corrective Action: Provider stated Area Coordinators, Program Specialists, and floating Directors will be working sites to help staff coverage. Operation Director and Executive Director will continue to assist at sites when in Rutherford county due to staffing issues.
