Summary of Follow Up and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, follow up, and complaint investigation. Technical Assistance Provided: 1. Contact TNPAL at 1-615-277-1688 or Support@tnpal.org regarding staff who are not linked to the agency in TNPAL and for staff whose trainings are not appearing in TNPAL. 2. As of 6/21/2023, all (new and previous to this date) educators as defined by Licensure Rules for Child Care Agencies are required to complete the TCCOTS Pro-Solutions Licensure Rules and Regulations Training found at, https://www.prosolutionstraining.com/tn/ under the tab labeled, TCCOTS Organizational Development and Administration. New staff are required to complete the training as a part of their orientation prior to having unsupervised contact with children. Previous staff have until 9/30/2023 to have the training completed. 3. All trainings for the QRIS cycle have to be complete by 9/30/2023. Refer to page 23 of the DHS Child Care Licensing Rules regarding annual trainings for primary educator and educator. 4. It was suggested that the agency have extra staff on the premises in the mornings to ensure all rooms are open when needed. Management reminded that the agency can only combine for the first/last 1 ½ hours of the day with children who are 2 ½ years old and older. 5. When infants are napping in cribs and playpens, make sure that the covers for the mattress is fit snugged. 6. Refer to page 48 of the DHS Child Care Licensing Rules as children are not permitted to wear teething necklaces, pacifiers, or any item around their neck or attached to their clothing that are potentially hazardous and associated with choking. Observation: Documentation was not provided to the parent regarding an incident that occurred 11/22/2021. It is required that parents receive documentation of incidents the same day of occurrence. 1240-04-01-09 Incident Reporting Corrective Plan: Management plans to make sure all parents get incident reports and have them sign stating they recieved it.

Adult:child ratios were discussed as the agency can only combine for the first 1 ½ hours of the day and the last 1 ½ hours of the day of children who are 2 ½ years old and older. Chart 3 was discussed with the director and the person in charge. 6. Meal/snack time supervision was discussed as children have to be seated while eating and staff must maintain direct supervision of the children. 7. Suggestions were provided regarding appropriate discipline methods. 8. A TAM referral will be submitted for staff to receive training and technical assistance in developmentally appropriate discipline methods, supervision, and behavior management. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The agency was not in compliance with adult:child ratios for the first 1 1/2 hours of the day. The agency was not in compliance with meal/snack time supervision.
