Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. While investigating Little miracles complaint it was determined that the complaint was invalid. The agency showed incident reports and warnings signed by the parent explaining that termination and probation were options if the behavior didn't change. It is also stated in their policy if parents do not pick up after 1 hour when called to pick up they have the right to terminate care. On 6/6/25 the mother showed up after 2.5 hours. The child did not say goodbye to the educator on her last day as stated in the complaint. She was picked up in the office by the parent end left around 12:30. This was verified via video footage. Interviews with staff showed no concerns about teachers laying children on their bellies for extended times nor any concerns about yelling or grabbing kids. There was an incident where a child got his finger slammed in the door as the class was coming in but the child was on the inside of the door with his teachers and class and was never unsupervised. An incident report was completed for this incident and the child did not seek outside medical care. Video footage was reviewed for a random morning in the dolphin class and they did not go out of ratio. Staff interviews did not indicate ratio problems. Daily logs and video footage was checked and diapers and potties were done every 2 hours. Interviews and files showed that incident reports are filled out as needed. The complaint was found invalid.

During an unannounced visit on 3/19/25, while observing video footage from 3/18/25 an educator was observed moving one child by pulling them on the carpet by their arm and leg and then moving another child by picking them up under the arms. Resulting in a scratch on the child.

One teacher was terminated and the others were retrained on name to face.
