There are 4 students who need the appropriate documentation of their immunizations. 0520-12-01-.05(3)(a) A program shall not enroll a child into care until the parent or guardian has supplied the program with completed application, Tennessee Department f Health Official immunization record (for children over (2) moths of age), and a health history. Staff records TBI/Sexual Offender/Vulnerable Person (Jennifer Hammer) 0520-12-01-.05(7)(f)(9) Staff records should include: verification of criminal background check and Vulnerable Persons Registry results. Staff physicals should be updated (Jennifer Underwood, Stephanie Sevier, and April Spurlock) 0520-12-01-.05(7)(f)(3) documentation signed by the examining physician, licensed psychologist, licensed clinician, Nurse Practitioner, or Physician’s Assistant verifying that the staff person is capable of safely and appropriately providing care for children in a group setting. The documentation shall be on file within ten (10) calendar days of employment or starting to work. An updated statement of each staff member’s physical health shall be obtained every third (3rd) year, or more often if deemed necessary by the Department. Staff Childhood diseases and health promotion document needs signature for all staff 0520.12-01-.07 (6)(c). Within the first (2) two weeks on the job and annually thereafter, each employee, including directors, teachers, substitutes, volunteers, and practicum students, shall receive annual instruction in (c) disease control and health promotion. All staff need to sign that they have read the Rules and Regulations 0520-12-01-.07(8) The program must maintain documentation that each employee has read the full set of all applicable rules. In addition , a copy of these rules shall be maintained in an area accessible to all staff. Post menus for parents inside/outside the classroom for those who don’t use the app “himama” 0520-12-01-.11(1)(h) In order for parents to be aware of the food their children are receiving, the week’s menus shall be planned and posted by the first day of each week and remain posted throughout the week. These menus shall be followed, although reasonable substitutions are permissible, if the substituted food contains the same nutrients. Any change shall be documented in advance of the meal. These corrections should be corrected in 10 days. Follow-up visit will be scheduled for 3/9/20.
