Observation: During a visit on 5/24/22 Licensing Consultant Jones observed Sign in and out sheets from 5/19,5/20, 5/23, and 5/24 and found the following: • On 5/19/22 a 20 month old was placed in the infant room. The maximum age for this room is 15 months. • On 5/19 the 4-5 year old classroom was over Ratio from 7:44-8:44 with up to 1:22. The ratio for this age is 1:16 • On 5/23 the 3-4 year old room was over ratio from 10:40-2:48 at 1:18. The ratio for this room should be 1:13 or 1:16 if only 4-5 year olds are present. • On 5/24 the 2 year old room was over ratio from 7:50-8:45 with up to 1:18. Observation: During the visit on 05/18/2022, Licensing Consultant Jones observed five classrooms out of ratio. In the first toddler room, the ratio was 1:7. In the second toddler room, the ratio was 1:8. The ratio for these two rooms should have been 1:6. In the 2 year old room, the ratio was 1:16. The ratio should have been 1:7 In the first 3-4 year old room, the ratio was 1:19. In the second 3-4 year old room, the ratio was 1:14. The ratio for both of these classrooms should be 1:13. Sign in/ out sheets were reviewed for 05/16/22 and 05/17/22. It was determined that there were multiple rooms out of ration on both of these days. The following was observed: On 05/16/2022: • From 7:35 am to 10:00 am, the infant room ratio was 1:8. The ratio should be 1:4 • Toddlers were combined with 2 year olds from 7:23 am -7:45 am • The 2 year old classroom was over ratio from 7:40 am to 11:40 am with up to twenty-two 2 year olds with two teachers. • The 3-4 year old classroom was over ratio from 7:30-8:41 with up to 25 students to one teacher then 2:24 for the rest of the day (the max for this age is 22) from 7:30-8:41 with up to 25 students to one teacher then 2:24 for the rest of the day. This Ratio should be 1:13 or 2:22. • The 4-5 year old classroom JrK B was over ratio from 7:38-8:04 with up to 23 to one teacher. The ratio for this age is 1:16. On 5/17/22: • Infants were at 1:8 and were out of ratio from 7:08-9:35 when the director took 4 in a stroller to the kitchen to fix lunch. The children were brought back at 10:45 and the class was over ratio at 1:8 till 12:06. The ratio for this age group is 1:4 • Toddlers were out of ratio from 7:15-7:35 with 1:12 then from 7:35-7:40 with 2:17 with toddlers and two year olds combined. The older two and a half year olds were taken to their class at 7:40 leaving the room over ratio up to 2:15. At 9:15 the younger twos were taken to their class and from 9:15 till the afternoon the room was at 1:8. Toddlers should not be combined with 2 year olds and the max ratio for toddlers is 1:6 or 2:12 • The two year olds were at 1:8 from 9:35 till 3:50pm. The ratio for this age is 1:7 • The 2.5-3 year olds were over ratio from 7:40 to 8:00 with 1:15 then 2:20 for the rest of the day. The ratio for this age group is 1:9 or 2:18 • Pre K A with 3-4 years was over ratio with 1:16 from 8:56-9:21. The ratio for this age group should be 1;13 • The 4-5 year olds in JK B were at 1:18 from 10:00-12:58. The ratio for this age group should be 1:16 • The early morning combining for the 3-5 year olds was over ratio from 7:27-9:16 with a max of 1:27. During an unannounced complaint visit on 04/15/24, LC Walker observed video footage of a lapse of supervision in the toddler 2 year old classroom. A child (22 month old) was left unattended for approximately 25 minutes in the classroom. While in the classroom the child was walking around looking at the door, playing in centers, under a table and sitting in the book area when one of the educators came in to get her 25 minutes later. Two educators (Paul Kar and Baxter) failed to follow the agency's supervision plan by not conducting roll call before leaving the classroom and upon arrival to the outdoor playground. During an unannounced complaint visit on 04/15/24, LC Walker observed video footage of the name to face roll call not being completed resulting in a lapse of supervision in the toddler 2 year old classroom. A child (22 month old) was left unattended for approximately 25 minutes in the classroom.
