Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint visit. Technical Assistance Provided: 1. Continue to make sure educators are supervising children and not using their phones unless there is an emergency. 2. Continue to follow supervision rules, name to face roll call. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans.

See Below for Detailed Information Observation: There was no designated staff to administer medication. 1240-04-01-.12 Health and Safety Corrective Action: The agency has created procedures for administering medication, including three designated employees that can administer medication. Observation: Medication was not stored inside a child proof container. 1240-04-01-.12 Health and Safety Corrective Action: The agency has two lock boxes for medication. One will be used for refrigerated medications, and the other one will be used for room temperature medication. Observation: A five year old boy received the wrong medication, per the educator's statement. 1240-04-01-.12 Health and Safety Corrective Action: The agency has created procedures as to how medication will be administered. This will be added to their annual training and for new employees during orientation. Observation: Educator had not been trained on administering medication. She stated in her interview she did not look at the name on the child's medicine bottle before administering. 1240-04-01-.06 Staff Corrective Action: All staff will complete medication training and incident reporting annually. New employees will also receive the same training. In addition, only designated staff will accept and administer medication. Observation: A five year old boy was given the wrong medication. The director did not self report the incident. 1240-04-01-.09 Incident Reporting Corrective Action: The agency has created incident reporting procedures to include during their annual training and for new employees during orientation.

Observation: Upon walking to another classroom LC noticed a child in another room by her cubby. LC checked to make sure no other adults were present in the classroom. LC asked the child who their teacher was. LC walked the child back to where the other children were finishing up lunch. 1240-04-01-.11 Supervision Corrective Action: When visitors are present the agency will ask that all questions are saved until one table has finished eating. This will free the other teacher to monitor the bathroom and door. The agency also talked to the child again about the routine.
