Observation: An educator left children in the infant classroom to come to the door to let an individual into the agency. An educator left a classroom to start lunch. With both incidents, classrooms were left out of ratio. An educator also had children as young as one year old grouped with children as old as four years old in the classroom. Another educator had a one year old in the 3 to 5-year-old classroom, and another educator had children as young as infant grouped with children as old as three years old. The children appeared to be able to go into any classroom. 1240-04-01-.22 Specific Requirements for Child Care Centers Corrective Action: Violation was corrected on-site when another educator arrived back to the agency and children were moved back to their original classrooms.

Corrective Action: The provider stated she would remind the parents to return the sheets every Monday after laundering them over the weekend and she will purchase some sheets to have extras on hand. She also stated she would replace the torn mats with new ones.

Observation: The infant room ratio was 1:5. The children were all under one year of age. The ratio should have been 1:4. 1240-04-01-.22 Child Care Ratio and Group Size Charts Corrective Action: The provider spoke with the teacher in the infant room and told her to only have 4 children in the classroom at all times unless there was another staff who could assist with more. CCR&R has scheduled a training for March 7th and plans to have a "coach" work with the staff at this agency.

OBSERVATION: On 3-26-18 it was observed that an infant was sleeping in a soft bouncy seat. This violates the safe sleep/soft bedding rule. 1240-04-03-.06 SUPERVISION/Safe Sleep, SIDS The issue was immediately corrected by the caregiver moving the baby to a crib. The Owner also completed a Plan of Corrective Action in which she stated she was going to retrain all of her staff on safe sleep procedures.
