Observation: The educator failed to call roll when she came inside from the playground in the 2.5-3s room. 1240-4-1-.11 SUPERVISION. Corrective Action: The educator has been talked to and corrected. Also the agency had a supervision training during nap time on June 5th to go back over the correct way to leave a room and return.

Observation: On April 26, 2018, during the annual assessment in the toddler class the assessor observed a lapse in meal time supervision. PE observed the video from the day of the assessment and saw at least two (2) toddlers crawling under the bucket-seat table with no teacher intervention. At one point it appears the lead toddler teacher saw the children and tried to get another teacher’s attention with no response. Assessor reports that at least two (2) children were seen with food, chicken nuggets & crackers. 1240-04-03-.06 SUPERVISION. Plan of Corrective Action: PE observed on 5.2.18 meal time supervision in this classroom has been corrected. Management also reports staff has since been re-trained on meal time supervision.

Summary of Complaint Investigation and Safety Plan Monitoring The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. LC provided guidance on baby gates. Baby gates that are operated with a foot pedal or the kind that can be easily opened would be considered accessible. Baby gates that require using fingers to unlatch them are not considered accessible. 2. Change report due 3/29. The director will complete the revisions and submit to LC. 3. LC discussed TNPAL. Each educator/staff will need to upload their CPR & First Aid certifications and their diploma/degree. When reviewing new educator requests, ensure start date is correct. 4. LC had the director remove the pacifiers from the pack n plays. LC informed the director if a child throws a pacifier in a pack n play that the child does not sleep in, the educator will need to change the sheet. No Violations were received.
