A report to the Department should have been made as soon as knowledge was received a child had been injured while in care. Educators did not complete an incident report and of a child falling on the playground.

The agency failed to report a serious injury to the Department on the same day of the incident. While reviewing the childcare center's video footage, it was observed that four (4) 1-2-Year-old children were eating a snack and educators did not provide direct supervision to the children during snack time. Educators were observed picking children up by their arms to move them to another area.
