Review of meal supervision. 2. Review of when to report serious incidents. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Agency does not have a copy of the incident report. Direct supervision did not occur during meal time. Agency did not report serious incident to the Department on the same day of the incident. Educator did not latch the manufacturer's safety restraint while the child was in the high chair.

Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. Importance of proper discipline techniques. Violations were received and include: 1. Food and Food Service 2. Supervision 3. Specific Requirements for Child Care Centers Correct Action Plan (CAP) completed. Violation details can be observed below. (3) Meal and Snack Time Supervision. (a) During meal and snack time, staff shall maintain direct supervision of children between six (6) weeks and five (5) years of age and maintain supervision of children between six (6) and nine (9) years of age. (b) During meal and snack times, educators that are providing supervision are prohibited from engaging in any activities unrelated to mealtime while children are eating. (1) Required Adult:Child Ratios. (a) Adult:child ratios in this paragraph shall be maintained by the child care agency while the children are on the premises of the child care agency, including outdoors or on the playground. (2) Choking Prevention. (b) It is the responsibility of the supervising educator to ensure that food is not accessible or served until it has been chopped, diced, cut or mashed and is appropriate for each child's age, and individual eating, chewing and swallowing ability. (c) Food shall not be given to a child until the supervising educator is able to provide focused attention to the child.
