Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. 1240-04-01-.09 Incident Reporting 1240-04-01-.06 Staff 1240-04-01-.22 Specific Requirements for Child Care Center 1240-04-01-.15 Program Language and Literacy Development A 22 month old child was spanked on the bottom by an educator. There was no incident report in the childs file. During an unannounced visit the two year old classroom had a ratio of 1:8. The ratio should have been 1:7. A 22 month old child was spanked by an educator and this was not reported to Department of Human Services ad to the Department of Children's Service.
