On 2-20-26, an unannounced visit to investigate a complaint was conducted by LC Macey Johnson (DHS) and Emily Rice (DCS), video footage from 2-10-26 was viewed. Educators Jamie Basham and Briley Payton were educators in the 6 weeks-30 month classroom. There was video evidence of both educators on their phones while children were eating and while feeding an infant a bottle. On 2-20-26, an unannounced visit to investigate a complaint was conducted by LC Macey Johnson (DHS) and Emily Rice (DCS), and video footage from 2-10-26 was viewed. Video provided evidence that Jamie Basham, an educator in the 6-week-30-month classroom placed a blanket on an infant while sleeping in the crib. Interviews were conducted with staff from the agency; two educators stated that they had witnessed what they described as possible abuse and/or neglect with infants and toddlers in the 6-week-30-month room on occasions in January and February. The staff did not report their suspicion to the Department of Children's Services, the Department of Human Services, or law enforcement immediately as required. Two educators are required for this number of children. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS (1) Required Adult: Child Ratios Corrective Action: The educators will receive training on active supervision from Child Care Resource and Referral and from the agency director. The agency director will develop a system to enable educators on the playground to communicate with staff inside the building as needed without coming into the building.
