The agency has been without required insurance coverage from 8-17-19. 1240-04-01-.05 OWNERSHIP,ORGANIZATION AND ADMINISTRATION. Corrective Action: The agency has been contacted regarding the lapse in required coverage and is in the process of getting insurance coverage re-instated. Coverage was reinstated 11-13-2019.
