On 8/19/19, during a complaint investigation, PE was informed of an educator who was terminated for using a raised voice and then spanking a child who refused to sleep. However, the center did not report this to the Department as required. The parent of the child was not notified either. No documentation of the incident is available. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT Corrective Action: Staff will be trained on the proper methods for disciplining a child in their care including tones and volume of voices able to be used.

On 8/19/19, PE was informed that 2 children under the age of 2 years choked on peanut butter and granola bars. Educators perform first aid procedures to clear the children's airways. Because of the first aid procedures administered, the children were fine afterwards. 1240-04-01-.13 FOOD AND FOOD SERVICE Corrective Action: The agency will change the menu to remove "Crustahbles" which contain large chunks of peanut butter and granola bars. Removal of such foods and future precautions when making the menu will be taken to ensure the safety of the children during meal times.
